Now more than ever, it is vitally important for our CCMC seniors to be in regular communication with their care team. We have taken all precautions recommended by the Centers for Disease Control to create a safe environment. 

For our patients, especially those who are high-risk and for those with high-risk family members, please embrace all precautionary measures recommended by the CDC.

• Wear your mask. 
• Practice social distancing. 
• Stay at home, when possible.
• Schedule your vaccine at CCMC.

Prevention and early intervention are key to maintaining a high quality of life for all of our patients. Our goal is to keep you safe and healthy and will happily provide the necessary healthcare through At Home Visits and/or Telehealth.
